Summary of CampThis is a fun soccer environment for boys and girls of all skill levels. If your new to the sport or an advanced player, our coaches will provide a fun learning environment for all. 
This camp is specifically designed for players who want to learn the fundamental skills of soccer. Emphasis on correct technique is maximized by small group instructions. There will also be small-sided games, providing more touches on the ball and a 'World Cup' format competetion to allow players to compete in a competetive environment. 
Campers will be split into age appropraite groups.
Cost: $275
Registration includes: Free T-Shirt, Water/PowerAid and daily time in the pool (A lifeguard will be on duty).

MEDICAL TREATMENT AUTHORIZATION

I/We being the legal guardian(s) of the above participant, authorize AH Elite Soccer Camps LLC and its agent’s permission to request medical treatment as necessary or to transport the minor to a medical facility to insure the well-being of the participant. I further authorize appropriate medical personnel to render such medical treatment as is necessary for the health of the minor. I agree to pay all costs associated with such medical care and transportation, and once the minor is in care of medical personnel or hospital AH Elite Soccer Camps, LLC shall have no further responsibility for the minor.

LIABILITY WAIVER

I approve of my child’s participation at AH Elite Soccer Camps, LLC and certify that he/she is in good health and able to participate in the program activities. I am attaching a statement explaining special physical limitations and/or required medication. (Please indicate if your child suffers from allergies, asthma, diabetes, restricted activities, or other health related issues.) In further consideration of AH Elite Soccer Camps, LLC accepting this application, I hereby agree to save and indemnify and hold harmless Florida Southern Men’s Soccer, AH Elite Soccer Camps, LLC, and Florida Southern College, its agents, employees and volunteers against any and all liability, claims, judgments or demands for damages arising as a result of injuries sustained by the applicant during or as a result of any activity/program given the participant of AH Elite Soccer Camps, LLC. Although Florida Southern Men’s Soccer and associates have taken reasonable steps to minimize risks for participants, I recognize that certain inherent risks are involved in soccer camp activities, including but not restricted to conditioning, and physical activity. Activities may result in the possibility of slips, falls, scrapes, and other injuries, which could result in scratches, bruises, sprains, strains, fractures, concussions, or even more life threatening hazards. I am aware of the risks to participants these activities entail; that this list is not complete and that other unknown or unanticipated risks may result in injury or death. I agree to resume responsibility for risks herein identified and those not specifically identified. My child’s participation in this activity is completely voluntary. I assume full responsibility of my child for bodily injury, death and loss of personal property and expenses thereof as a result of those inherent risks and dangers.
